Prepare release 14.3.0 #486

Most of the code in this PR has already been reviewed with the exception of __init__.py. Please make sure the release version matches the title of this PR.

Allow for different patch public release to be specified
Fusion entrez gene id must be integers
Support initial SV format
Don't include release version in filenames
