In a Project, I go to Insights and click the New Insight button. I fill out the form for everything except the attachment and click "Create New Insight". However, it stays on the form and doesn't seem to do anything. I clicked the "Create New Insight" button 3 more times and it still stayed on the form. I then added a Tool as an attachment and clicked "Create New Insight", at which point I was redirected to the main listing of Insights where I saw 4 of the Reports without an attachment and 1 Report with the attachment:
