Adds confirmed state
Fixes bug with Blockcypher adapter (for testnet)
Adds user_id to payment (could be made more generic like sender_id and sender_type)
Switches to Coinbase ticker with Gemini as fallback (more reliable, bitcoinaverage crashed on me a couple times)
