Hello,

Love the app, can you tell me how to add additional fields when creating an issue please? Thanks Ross

That part is LWC and will need to be added by me. There is, unfortunately, no feature to dynamically add those fields.

You can always take the source code from this repo and configure a component as you need. Of course, that will not be AppExchange listing anymore.
