Additional field cleanup and permissions additions #79

Found fields that are not used. Added permission set.

Thanks for these change. Unfortunately, I didn't check all the tests before I merged into master. So master branch is not failing and I cannot promote this to packaging org for new package version for AppExchange.

ViewSurveyController_Test is the only class failing. I know you made many changes there so you are familiar with those. Can you please look into this?

Nvm. I figured it out. I wasn't using it correctly when creating standard controller.
