No notifications on android wear for incoming calls

What steps will reproduce the problem?
1. Have an android wear device linked to the phone with CSIPSimple installed
2. Receive the incoming call on the phone

Expected to see notification pop up on the android wear device about the 
incoming call. Currently there's no notification.

Tested on Nexus 5, Android 5.0, CSipSimple 1.02.00, Android Wear 1.0.4.1580939.

Surprisingly enough, the missed call notification is displayed on Android Wear 
(because I guess it goes into notifications) but not the incoming call 
notification.

Oh, sorry, forgot to add the following.

The notification for the incoming cell call shows the caller's avatar and an 
option to slide right to hang up and slide up for a few pre-set text responses. 
I believe there used to be an option to slide from right to left to answer the 
call on the phone, but it doesn't seem to be the case now.

Also experiencing this with an Asus Zenwatch w/ Android Wear 5.0.2 linked to a 
Nexus 5 with 5.0.2.  No notification for incoming calls, just a notification 
that a call was missed.
