I have some suggestions, tell me if you agree with them:

• Currently, when you click the submit button nothing happens, I would say we should right now redirect on the same page and show an alert(), so that we can check if it works fine
• Next, if the person deselects the Terms and Conditions, we should show a message saying required. I think its pretty easy to implement and pretty much is the same as you did before
• Also when I input something correct in the Email and Zip input, it still shows the message(it works fine for contact number)...
• Also, at last would be good to shift the Get Books to the home page like we have done for Donate Pages(Rename Get a Book to Get Books)
