Step 7: Maintainer's guide

This pull request adds a maintainer_guide.md to guide maintainers of open source projects. This could be included in a public-facing way, but may be best kept privately in this repository.

Use this document to formalize expectations for maintainers and help everyone learn how to maintain well.
