[Urgent/fix] Adapted code to the new baxter_collaboration #15

Basic functionalities are the same, but now the code (especially the computer vision part) is more refined and clean. There are some other minor things here and there to fix, but I have to wait for the trac_ik developers to see if they are going to be able to fix their code or I'll drop it and move to some other solver.

Plus, the code has been reduced by a whopping 600 lines!
