Three options appear available

Use a physical switch driven by some physical action Peter can still perform - e.g. similar to Stephen Hawking's cheek switch
Listen for a noise Peter can still make such as the teeth clacking Peter is currently using
Use a mission-critical non-stop eye tracking technology which is not subject to technical 'blips' that afflict current ACAT/TOBII solution
