Clarify the use of different age comparisons and related parameters #648

We have parameters relating to various age groups, for example diseaseParameters.pSymptomaticCaseAdult is used for people who are >20 and <70 years old. In order to avoid any confusion or ambiguity, it might be useful to rename some parameters (or possibly add documentation or do other refactoring).
