Output Tables Written to Individual Files as Opposed to Single File #753

The model outputs should be being written to a single HDF5 file for each category in cases_deaths, dynamics_summary, dynamics_time_series but are instead being written to single files. This is obviously a bug since the recent change to the API usage.
