Generate ToC: make items deletable from preview #537

Make it possible to delete single items from the preview box of the expanded Generate ToC option panel. When an item is deleted it should not be included in the generation of the ToC.

Just add a clickable "x" to every item in the preview box.

EDIT: Needs #573 to be done first!
