Once the player has completed the jump puzzle they can simply walk into the lava to be teleported back to the start rather than going through it again. This is an issue in other places with other puzzles too.
One solution would be to have "checkpoints" scattered through the level that set the player's respawn point when they are collided with. This can help with #36 also.
