resolves #209

This PR accomplishes the following:

Deletes pull_request.md in .github/ISSUE_TEMPLATE folder.
Creates pull_request_template.md in .github folder.
Adds user story and issue titles to front matter for user story and issue templates.

I modified this existing PR to instead achieve the goal of deleting all templates. They've all been added to the .github repo where they can be centrally managed for all repos.
