Clean up #2

@kghbln @jchapin Incorporated changes from #1, added composer.json, and some minor adjustments to make at least adhere some SMW standards. Could someone test this if it still works because I don't use SES/SF.

Confirming that this solved #1 . I just installed "Version 1.0-alpha" on the sandbox and the production systems. This issue is addressed. There's another issue that popped up with the {{#signupfields:}} parser function, but I need to thoroughly investigate what's going on in the new version before opening up another issue. Thanks for the updates!
