Feature test for viewing the summary page for a rebate form in the iPad UI

adds a helper to match (approximately) the currency formatter used in the iPad UI
fixes up some dodgy values in the rebate_form factory
adds a missing ENV key to env-example

Only tests the summary page. Further tests for the signing workflow will follow.
