Adds the ability to decline and restore rebate forms
can only decline before the form is processed
declined forms are hidden in the existing form lists
declined forms are visible in the new 'declined' tab
decline a form by clicking on the X in the top right nav on the form show page
undecline a form by clicking the tick which replaces the X
to decline or undecline a form you must provide a reason
when a form has been declined or undeclined the MOST RECENT ONLY action will be shown at the top of the form show page, describing the action, who did it, when, and what reason they gave.
Process and Unprocess links have been replaced by right arrow (process) and left arrow (unprocess) icons which appear in the top right of the form show page. (Karyn's request)
