I experienced this in a modpack, and have been able to replicate it in a separate modpack with only Jade, Apotheosis, and their dependencies. The modules I have disabled are the Enchantment module, the Spawner module, the Garden module, and the Chronicle of Shadows being given upon world creation. While I have modified the config of the Adventure module in the larger modpack, and have not done so in this test modpack, the fact that the issue appears leads me to believe it has something to do with one of the modules I disabled.
The issue does not appear when all modules are active. And, while not a game-breaking issue, it is incredibly annoying. The issue can be seen in the image below.
