Added Date-time entry field to form, along with adding the entries to table #6 #38

Modified the index.html and app.js file to add the date-time entries in the fourth 'Date & Time' column. Hope that fixes both issue #6 as well as develops upon it. Kindly awaiting your approval, @Sharmi-1999. Thanks.
