Extension: add "quick disable" for extension #723

This lets extension users more elegantly handle error cases.

We should put an extension icon in the chrome menu bar and via right click, allow an option for "disable video radar on this page."

This is similar to the way adblock works and should lower user frustration with the extension if they run into issues.
