I have crafttweaker and i can't seem to figure out how to make it require the item in the middle of the rituals instead of just the outside one.

Not sure if you're the same person as the one who asked this question in the MMD Discord channel, but what you're looking for is the Infusion Ritual, not Creation. Infusion Rituals utilize the altar slot.
