Update the README:

Be clear that configuring the library based on environment variables is recommended, and document all supported environment variables.
Focus less on implementation details.
Add StatsD.histogram which was missing for some reason.

Update the CHANGELOG:

Add a note about the STATSD_PREFIX and STATSD_DEFAULT_TAGS environment variables which are now supported to configure the library.
