Death on survival is ok, im not sure why this did happen

That sounds like you're using Frackin Universe. Frackin Universe for whatever reason turns the perma death difficulty for characters into not perma death. Something that needs a patch for every single race because starbound normally expects that difficulty to not need respawning animations.
