Update to PIN 3.15

• Update all deprecated functionality to PIN 3.15
• Add SQLite in-tree since we may no longer link against certain external libraries due to restrictions in PIN 3.x
• Add syscall wrappers for functions that are required by SQLite but not provided by PIN CRT
• Remove self-modifying code functionality and documentation because it appears to not be supported anymore, and I didn't spend the time to figure out why, or what should replace it

Notes:

• Didn't update Debian installation guide because I don't run Debian
• Does making SQLite a git submodule make more sense than pasting in the files?
