As a developer, I would like to gather the Mathematics Department faculty and classroom information to start developing new UI's for plaques once scanned.

Acceptance Criteria:

A Google form (passed down from last year's team) is sent to the head of the mathematics department to distribute amongst faculty. Include a deadline for November 15th to ensure that we can use it for Sprint 3

Definition of Done:

Acceptance Criteria have been met
Signed off and moved to done by the Product Owner
