Description:
When standing at max range of Halo it will not hit your target. This happens just with base Halo but behaves similarly incorrectly with Archon or Phantom Reach that increases the range.
Distance checking is a bit awkward but this appears to be my experience:
28 yards: 1 hit
29 yards: no hits
30 yards: no hits
If we swap to Archon to get the +10 yards from Power Surge:
38 yards: 1 hit
39 yards: no hits
40 yards: no hits
With this we can extrapolate out what Phantom Reach would do:
Non-Archon: 28 * 1.15 = 32.2
Archon: 38 * 1.15 = 43.7
Depending on how the game rounds this means when standing at max range with Phantom Reach and as Archon you could potentially be up to 3 yards short of Halo hitting your target.

The following is without Phantom Reach, you can see I am in range of Psychic Horror (30 yards) but Halo never hits the Training Dummy
The following is with Power Surge to get a 40 yard range, you can see it is also not hitting the target:
