Creating pull request after solving created issue #653

Solving issue#653, including fixing bug that previously allowed to use and fill in partial/invalid phone number; form adjustment for queue page as well

This PR has way too many changes, this PR says 48 files changed. I think the git history got messed up for some reason. Could you pull the master again, create your new branch from there, then make changes again and raise another PR?

I am to take the blame for this. I rebased prod to master and this made some history rewrite.

I am very sorry about this. @danielvydra Let's try to see if merging upstream master into your branch goes smooth without conflicts, otherwise try cutting from latest master and cherrypicking your commit f325e92d4d5a6798bbea83d4e6121a858937bfb2.
