I've been trying to figure this out all night. The crash log in question is finally my attempt with a clean launch of the game and only Forge and this is installed, but the exact same issue happens.
In creative it's perfectly fine and doesn't crash at all, but in survival as soon as I start to break a block it crashes before the little box can even show up that something is breaking.
