Coding style - adopt standards

• indent with spaces, not tabs
• block braces on the same line as the opening command
• jsDoc for private / inner methods too
• compiler directives (@constructor) and method visibility (@private)
• avoid calling .length in loop exit condition (possible performance isssue)
