The Table of Contents #2

@jpkeisala has broken the ice (thank you!) and submitted the very first draft of the Table of Content. We can use this issue to track comments and ideas.

Thanks to the contribution of many we have now a good TOC that can be used as a solid starting point upon which we will always be able to build, obviously. For now, I will close this issue.
