Edits to Appendix A #21

I'll work on this in chunks. When you say you want to tie the episodes to the book a bit more, do you mean if it mentions something discussed in a chapter that we reference that? Or did you have other ideas for this? Just so that I know what to keep an eye out for.

Yes. But in addition to that, if an episode fits really well with a chapter, we can move the QR code to that chapter. For example at the end of the chapter on Eclipse attacks, we link straight to the episode on the Erebus attack. So that episode is no longer mentioned in More Episodes.

I plan to merge this after #22 because the new URL's are probably going to cause a merge conflict (which I'll fix).
