Added github templates for issues and PR

Same as in vagrant-hashistack.

LGTM, probably we could just downstream these changes on sync to modules

I agree. The reason it got created PRs for each module was that there was a discussion earlier about not wanting to have this workflow forced on the users of the module. But in the future if we want to add/remove/fix the github templates we could just change it in this repo and sync with modules later on.
