Need a system through which multiple prescriptions of different doctors could be stored in database. A patient page where all information about a particular patient could be retrieved in proper way in both the hospitals, primary as well as city.

In city hospitals all doctors could review about patient and access all its logs, also can provide his/her own prescription.
Village hospitals can add a new patient, update its information and delete the unrequired prescrptions.
