Theres much more systems which is not completely paused. Does it really matter for you?
Keeping this as the oldest currently open note of the problem.
Unturned has kind of a mess of using real time rather than in-game time. Back in 2.x the time was synced between all servers which carried over to 3.x. Adding pausing on the pause menu and /time and /cycle commands didn't update all the existing code for that.
I should definitely go back and fix this up at some point, but no ETA.

When we pause game in singleplayer everything stops but not time. Singleplayer time (day & night cycle) should be also stopped when we pause game
