Forgot Handle/Password request form needs some cosmetic fixes

When a user clicks on the link that says "Forgot your handle or password?" it 
goes to 
http://youdescribe.ski.org/test/forgetPassword.php

1. The title of the page needs to match the heading. Currently the top heading 
offers the possibility of changing the handle which is not actually on offer. 
The heading should be changed to match the title.

2. The button says "Bo." This is not terribly descriptive. I would prefer 
something like "Send E-mail" or "Send name and password info."

3. We have to be consistent about our use of the terms name, username, and 
handle. I think the term Username is most clear and common. We should work on 
this here and elsewhere.

Confirmed that this is fixed in the current release 
(YouDescribe-release-20131220).
