Create form for user signup

Users should be able to sign up once and then sign in thereafter to save time on the farmer side.

This should store to firebase and should include:

• donor name
• address (street address, city, state, zip - 4 fields)
• email
• phone

This works now, but I would like to link the FB updating to an authorized user, so the db isn't so wide open.
