Add territories #1059

What it says on the label: instead of 'state' everywhere, we want to say 'state/territory' and add territories to the db.

Issue #62 Include territories in list of states?

@frankduncan and I have reviewed each other's commits, and it all looks good to us. An earlier version passed the integration tests, and it deploys locally, and we would like to merge so that we can set up a demo for tonight, so we're going to skip the CI line and merge now.
