As admin, uncheck 'same as above' but can't edit fields #1094

When filling out a Pharmacy application as a service admin, on the 'Organization Info' page, unchecking "same as above" for the billing and 1099 addresses does not let you enter text into those fields:

However, this does work as expected when you are logged in as a provider:
