Show author and timestamp on enrollment notes

As demonstrated in these screenshots, a service admin can write and view notes on enrollments. But the PSM doesn't show who wrote a note or when. It would help the admin better understand and interpret notes on enrollments if the PSM automatically included the note's author and timestamp for each note.

This metadata does show up in the Notes interface at cms/provider/enrollment/jump?page=Notes when editing an individual renewal or enrollment, per the screenshot below, so I think we should also show it in the dashboard interface.
