Refactor 'findMyProfiles' method to be more DRY #974

I was looking at this method as I worked on #949, and thought it needed some refactoring. Tested by checking to see that the list of profiles on the "My Profile" page was the same before and after the changes.

Issue #949 Give providers the ability to "edit and resubmit" applications

Ah, thanks @jasonaowen for the info and pointers about NPI proxy being about linking to external accounts. I wasn't sure what that was for. (@frankduncan this may be of interest for #949 and friends.)
