When doing #254 we realized that somewhere along the line two versions of the same table got made with identical seed data. We should drop one of them and consolidate the code so that it only uses the chosen table.
This is a sub-issue to #57.
This will require #980 to be merged before it can be completed, since both involve seed data.
711 looks related to this task -- I don't believe that implementation of Categories of Service is in scope for this issue; this will be a 1:1 replacement of currently implemented functionality.
