Break apart provider and enrollment in other entities #993

The next of the giant refactoring of how enrollments and profiles work with each other. This splits the various other tables and each of them choose a side, are they part of an enrollment, or part of a profile.

@frankduncan noted in Zulip chat just now that this should also have PR #1003 merged first for safety (note that PR #997, another prereq for this, also depends on PR #1003).
