PointToPoint fixes #189

Move the location of the waypoints to between start and end
check the 'include waypoint' checkbox given 1) the he checkbox is not checked 2) the user clicks on the popup 'setwaypoint' button
if the next waypoint input field is empty, 'set waypoint' should just fill that waypoint without including another empty waypoint input field
add form submission integrity check function: user has to fill start, and along with all visible waypoint input fields to submit the form

fix bug associated with clearing waypoints (currently clicking on the x button will result int the input field of the waypoints beneath the selected waypoint's to be cleared too)
