Improvement of contact us form #240

The current contact us form contains only the feedback field, but a normal contact us form also includes the following important fields

**1. Name

  1. Any contact information phone or email of the person who sends the feedback**

I would like to implement these fields and I will also add inline validation in the form to prevent it from receiving wrong data
