Fix images with titles #45

Adds quotation marks around image titles and escapes any quotations inside them.

Seems to work ok when tested on https://docs.google.com/document/d/1hHAx92e89YQfBXT96C7BLiuMipcl6LOszGlG2L7rrZo

@mruwnik Pretty sure this is simple enough that I can just #sendit but wonder if you happen to know of any possible oddities with how this will behave
