Update handling user handles in Team table, so that updating stamp counts works again #303

Discord switched from user handles (<UserName>#1234) to new unique user names (or whatever they call them), so Stampy no longer updates stamp counts in the table correctly.

Also, it's probably worth doing this together with #205
