Dense scanner fix #1071

  1. Added a opal workaround indication flag to functions to optionally enable the software workaround for the broken dense scanner in opal.
  2. Added code in build_tb.py to generate GLB input file that contains the (2, 0, dim_size) input pair for the updated dense scanner.
