Cleanup Code and Project Structure

:recycle: Current situation & Problem

The current project includes a lot of different files from the CardinalKit Template Application that are not needed for the limited system demonstration.

:bulb: Proposed solution

This PR removes unneeded files and cleanups the code.
Makes it easier for developer to inspect the code by removing unused UI and unit tests
Surfaces the Xcode Project at the main level of the project
Renames the App to HealthGPT
