Many Turkers have made user mistakes on surveys-- not filling out surveys, not noticing instructions. This is due, in large part, to UI issues on our part, including:
Text that is too small, or doesn't have enough contrast with the background. In particular, I have received numerous complaints that text boxes are not visible (on many computers, they look like a white box on white background), and people don't know where to type, or even that they are expected to type something.
Text input appears very similar to text instructions.
Text boxes are small and narrow, and it's not possible to create an 'essay box' type input that allows for the kinds of longer responses we may be interested in.
Contrast is not consistent. People ignore text boxes because radio buttons are too high-contrast, but text boxes are too low contrast.
